19-29 years old – $60/month
30-49 years old – $75/month
50-65 years old – $90/month
66 years and over – $110/month

There is a one-time, non-refundable enrollment fee of $150

After 2 months of non-payment, you will be automatically discontinued from the clinic.  Attempts will be made by Heron Creek DPC via contact information you provide to make you aware of the missed payment.  Once discontinued, your membership may be filled by an individual on our waiting list. We can discuss the possibility of re-enrollment.  A fee of $250 will be applied.

$150 per visit for members only (within 20 mile radius of clinic)

All labs and medications will be charged at wholesale prices plus a 15% administration fee.
